• Excellence in Chronic Care Management Program: Improve care, decrease expenses across six major conditions- The excellence in Chronic Care Management Program encompasses six chronic conditions: congestive heart failure, chronic obstructive pulmonary disease, diabetes, asthma, hypertension, and coronary artery disease. This program measures the quality and cost of care delivered in physician practices. Physicians who meet certain performance criteria are rewarded with a variety of incentives, often including cash payouts. Employers and health plans are rewarded with healthier members and lower costs of care.
