The objective of this report is to summarize the results of a comprehensive data analysis and identify opportunities for improvement both in terms of 1) lowering costs and 2) improving quality of care using the unique features of the Prometheus Payment model’s Evidence-informed Case Rates (ECRs). After summarizing an overall view of the results of the data analysis, this report will identify areas of opportunity in your data and extract information that will make your data actionable. The prime focus of the report is to understand the sources of variation in your current data, both in terms of cost and quality, and provide recommendations on how to move forward.
